If you require any vaccinations relating to foreign travel you will need to fill in a Travel Form with the details of your travel arrangements.
This will include which countries and the areas within countries that you intend visiting, any previous travel vaccinations and general health questions. This will enable the nurse to determine what vaccinations are required.
You will need to fill in this questionnaire which must be returned to the surgery. The Nurse will then contact you to explain what needs to be done next.
This needs to be done at least 6 weeks prior to travel.
If you are travelling within the next 6 weeks then you will need to attend a pharmacy travel clinic such as Hollinswood Pharmacy or Boots in Telford Town Centre.
There will be a charge for this.

It is important to make this initial appointment as early as possible - at least 6 weeks before you travel - as a second appointment will be required with the practice nurse to actually receive the vaccinations. These vaccines have to be ordered by prescription as we do not hold them as a stock vaccine. Your second appointment needs to be at least 2 weeks before you travel to allow the vaccines to take effect.
Please note that although the Typhoid vaccination is available at NHS expense i.e. we do not charge a fee for them, They are obtained by the patient on an FP10 prescription. A prescription charge (Currently £9.90) is payable to the pharmacy unless the patient is exempt
Some travel vaccines are ordered on a private prescription and these incur a charge over and above the normal prescription charge. This is because not all travel vaccinations are included in the services provided by the NHS. There will be a charge for the following immunisations:
- Hepatitis B
- Meningitis ACWY
- Yellow Fever (Done at designated clinic please see: https://nathnac.net)
- Japanese B encephalitis
- Tick borne encephalitis
- Rabies
- Bacillus Calmette-Guerin (BCG)
Please note we do not do MMR vaccinations for travel in GP Practice. You will need to attend a travel clinic in Boots etc.
Yellow fever vaccinations are done at specific Yellow Fever clinics and not in General Practice.
Yellow fever vaccine needs to be given a minimum of 10 days before travel and typically costs £65 - £85 per dose. Local Pharmacies that offer this service are Hollinswood Pharmacy and Muxton Pharmacy.
The cost of a private prescription is £15.00 per patient and they will also incur pharmacy costs.
Malaria tablets for adults are now an over the counter medication and can be bought from your local pharmacist. Malaria for children is a prescription only medication and will incur a private prescription fee. The Nurse will need the child's age, height and weight before a script can be issued.
